If you currently use Optify for Salesforce and are upgrading from a version earlier than 1.43,  you need to upgrade the Optify Lead Layout, since Optify for Salesforce updates the information on this object.

To set up page layout for Optify lead object:

1. In the sidebar on the left, look for App Setup and click the Create button underneath.
2. Underneath Create, click Objects.
3. In the Custom Objects page, click Optify Lead.
4. Scroll down to Page Layouts, or click the Page Layouts link at the top to jump to that location.  Then click the Edit link to the left of the Optify Lead Layout page name.
5. Add all the fields shown below to the screen layout.  The new fields are outlined in red.
